Hi Eric,

Yep, that looks like a CSS issue. Perhaps the Crowdstack CSS didn't load fully? I'd recommend having the user clear cache and retry the request. The other thing to check is to make sure the Internet connection is stable. If it's not, the main page may load and the CSS request could fail (e.g. timeout).

I'm assuming this is the only user with this issue? It will be helpful to know if any other users are experiencing similar issues. If the problem persists for the user, it would be helpful if we could get more details such as device, operating system/version, and browser/version. The user may also want to try another device to see if it makes a difference.

I did a cursory look through our error reporting dashboards and don't see any evidence of server-side issues that may have contributed to this problem.
